Cardiovascular patients with covid-19 in intensive care: a systematic review

Resumo

OBJETIVO: Analisar na literatura, a prevalência de doenças cardiovasculares (DCV) e o desenvolvimento de disfunções cardíacas ou vasculares, em indivíduos internados em unidades de terapia intensiva (UTI), devido Covid-19.

MÉTODOS: Revisão integrativa da literatura, realizada nas bases de dados BVS: BDENF e LILACS; PuBMED: MEDLINE; Web of Science e CINAHL. Critérios de inclusão: artigos na íntegra, em português, inglês ou espanhol, publicados a partir de 2019.

RESULTADOS: Foram selecionados seis artigos e elencados três temas centrais: pacientes com DCV prévia; pacientes que desenvolveram doenças cardíacas ou vasculares durante a internação; e, pacientes que cursaram com alterações de biomarcadores de lesão cardíaca e de degradação de fibrina. Observou-se prevalência de DCV entre 0 e 29% dos estudos, como doenças coronarianas, arritmias, disfunções valvares e ventriculares, e a incidência de DCV entre 2,6 e 59%, apresentando alterações elétricas, funcionais, estruturais e choque cardiogênico.

CONCLUSÕES: Identificou-se percentagem importante de pacientes com Covid-19, internados em UTI, que apresentavam DCV prévias e que desenvolveram DCV. Entretanto, até a finalização do presente estudo, não havia pesquisas em que 100% da amostra possuísse DCV prévia, o que seria importante para a compreensão dos possíveis acometimentos sob ela. Assim como, pesquisas que especificassem os tipos de acometimentos desenvolvidos, nomeados apenas como “lesões cardíacas” ou aumento de biomarcadores. Como limitações do presente estudo, ressaltamos que novas pesquisas foram realizadas após a sua conclusão, que certamente agregarão melhor conhecimento. Desta forma, mantém-se a preocupação da gravidade em que a Covid-19 pode acometer pessoas com DCV prévia e suas manifestações cardiovasculares.

ABSTRACT

OBJECTIVE: Analyse in the literature, the prevalence of cardiovascular diseases (CVD) and the development of cardiac or vascular dysfunctions, in individuals hospitalized in intensive care units (ICU), due to Covid-19.

METHODS: Integrative literature review, carried out in the databases: BVS: BDENF and LILACS; PuBMED: MEDLINE; Web of Science and CINAHL. Inclusion criteria, full-text articles, in Portuguese, English or Spanish, published from 2019 onwards.

RESULTS: Six articles were selected, and three central themes listed: patients with previous CVD; patients who developed heart or vascular diseases during hospitalization; and patients who presented alterations in biomarkers of cardiac injury and fibrin degradation. A prevalence of CVD between 0 and 29% of the studies was observed, as coronary diseases, arrhythmias, valvular and ventricular dysfunctions, and the incidence of CVD between 2.6 and 59%, presenting electrical, functional, structural alterations and cardiogenic shock.

CONCLUSION: An important percentage of patients with Covid-19, admitted to the ICU, who had previous CVD and who developed CVD were identified. However, until the end of this study, there weren’t studies in which 100% of the sample had previous CVD, which would be important for understanding the possible impairments under it. Even as, research that specified the types of involvement developed, named only as “cardiac injuries” or biomarkers increased. As limitations of this study, we emphasize that new research was carried out after its conclusion, which will certainly add better knowledge. This way, the concern about the severity in which Covid-19 can affect people with previous CVD and its cardiovascular manifestations remains.
