"Near miss mom": important indicator of the care provided to the pregnant woman

Resumo

OBJETIVO: O termo Near Miss foi introduzido no estudo da morbidade materna fazendo referência à situação onde a mulher apresenta complicações potencialmente letais durante a gravidez, parto ou puerpério, somente sobrevivendo devido ao acaso ou ao cuidado hospitalar. O presente estudo teve como objetivo verificar as características das mulheres e as variáveis associadas ao quadro de quase morte.

MÉTODOS: Estudo observacional transversal, tipo levantamento, descritivo e analítico, realizado por meio do Serviço de Arquivo Médico e Estatística (SAME) do Hospital Maternidade de Interlagos, localizado na região sul da cidade de São Paulo no ano de 2019. Após a coleta foi realizada estatística descritiva a partir do Teste de Mann - Whitney (Siegel), Teste de qui quadrado (Siegel) e Teste G para análise completa dos dados.

RESULTADOS: Demonstrou-se que a idade materna, a idade gestacional o Apgar de primeiro e quinto minuto não contribuíram para aumento de complicações e casos de Near Miss Mom. Entretanto, o número de filhos, abortos, pré natal inadequadro e parto cesariano mostrou ser significativamente maior nos casos de gestantes que chegaram ao Near Miss. Além disso, a presença de comorbidades em geral não foi significante, entretanto, encontramos um padrão nos casos em que 44,4% constatamos problemas pressóricos, 11,1% era tabagista ou usava algum tipo de droga ou álcool, 7,4% tinha Diabetes e 7,4% infecções do trato urinário de repetição. Além disso, foi possível observar que as principais causas da morbidade materna foram as mesmas que foram apresentadas na literatura - crise hiertensiva, infecções, e hemorragia, além da crise glicêmica e descolamento prematuro da placenta que também se destacaram no estudo.

CONCLUSÃO: As principais características das mulheres que chegaram ao quadro de quase morte foram multigestas, sem o pré-natal adequado e que tiveram parto não normal.

ABSTRACT

OBJECTIVE: The term Near miss was introduced in the study of maternal morbidity making reference to the situation where a woman presents potentially lethal complications during pregnancy, delivery or puerperium, only surviving due to chance or hospital care. The present study aimed to verify the characteristics of women and the variables associated with the near-death condition.

METHODS: Cross-sectional observational study, survey type, descriptive and analytical, carried out through the Medical Archive and Statistics Service (SAME) of Hospital Maternidade de Interlagos, located in the southern region of the city of São Paulo in the year 2019. After collection, descriptive statistics was performed from the Mann - Whitney Test (Siegel), Chi square test (Siegel) and G test for complete data analysis.

RESULTS: It was shown that maternal age, gestational age the first and fifth minute Apgar did not contribute to increase in complications and Near Miss Mom cases. However, the number of children, miscarriages, inadequate prenatal care, and cesarean delivery were shown to be significantly higher in cases of pregnant women who reached Near Miss. Also, the presence of comorbidities in general was not significant, however, we found a pattern in the cases in which 44.4% found blood pressure problems, 11.1% were smokers or used some kind of drug or alcohol, 7.4% had Diabetes, and 7.4% had recurrent urinary tract infections. Furthermore, it was possible to observe that the main causes of maternal morbidity were the same as those presented in the literature - hypertensive crisis, infections, and hemorrhage, besides glycemic crisis and premature placental abruption which also stood out in the study.

CONCLUSION: The main characteristics of women who reached near death were multigestion, without adequate prenatal care, and who had a non-normal delivery.
